people come to Cedar Crest for short-term
services often referred to as subacute
rehabilitation or post-acute/post-hospital care.
Cedar Crest has two units completely dedicated
to caring for short-term patients with acute
illness, injury or disease – often following
hospitalization – or for those requiring
post-surgical rehabilitation. We have
strategically prepared ourselves to excel in
caring for the needs of these complex patients
with increased staffing, advanced education, new
technology and state-of-the-art equipment and
treatments.
Cedar Crest’s subacute program offers services
to clients following orthopedic surgeries or
related accidents, stroke, neurological
disorders or hospital stays for any chronic or
acute illness. In general, all patients benefit
from patient-centered care and an
interdisciplinary team approach.
Tailored Therapy, Optimum
Results
Cedar Crest provides
utilizes primary nursing care and
tailored therapy services for a patient
until he or she is well enough to be sent home
to complete recovery. Specialized programs, such
as joint replacement rehabilitation, cardiac
care, or cancer care provide goal-oriented
treatment with frequent patient assessments and
clinical management until the condition is
stabilized, or treatment course has been
completed. Treatment plans include physical,
occupational and speech-language therapy, along
with nursing care oriented to the needs of
rehabilitation patients. Our comprehensive
approach to care planning includes home
evaluation if needed, patient and family
education, and coordination of home care
services to be received after discharge from
Cedar Crest.
