People may be referred to Cedar Home Health by discharge planners at hospitals, a physician, a relative, friend, or by patients themselves. We are happy to discuss eligibility or payment for homecare needs and will work diligently to streamline the admissions process.

At other times, an answering service will refer calls to our 24/7 on-call registered nurse who will return your call as soon as possible.

To facilitate the referral when you request care by phone, please have the following information available:

• Diagnosis
• Medications
• Insurance Information
• Admission/discharge Dates
• Physician’s name and phone
• Plan of Care
• Primary caregiver and phone numbers

To make a patient referral via fax, please print this referral form and fax back to us at (401) 944-6241. Our staff will contact you with 24 hours of receiving this referral form. In order to protect the privacy of the patient, please do not email this referral information.

Assessment Process

Once the initial referral information is received, a registered nurse from Cedar Home Health will make a personal visit to the client’s residence to discuss medical care requirements, answer questions and to create an individualized plan of care with input from the client.
